IMPORTANT INFORMATION ABOUT APPLYING FOR AN ACCOUNT
To help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each customer who opens an account.
What this means for you: When you open an account, we will ask you for your name, address, date of birth, and other information that will allow us to verify your identity. We will also ask to see a copy of your driver's license or other primary or secondary forms of identification from the list below.
• Please fill out your form carefully. Incomplete applications will delay processing and may be returned.
Acceptable Primary Forms of Identification
• Valid U.S. driver's license (in or out-of-state), showing your current address
• Valid U.S. passport
Acceptable Secondary Forms of Identification
• Social security card
• Health insurance card
• Birth certificate
• Valid credit card
• Voter registration card
Note
If a primary source of identification is not available, two secondary sources may be used, so long as one of them contains a photograph.
